Insurance Information

We participate with most local and many national insurance plans. However, it is your responsibility to understand whether your plan has limitations on which doctors you can see and the services that are covered.

If you provide us with accurate insurance information, we will gladly submit claims to your insurance carrier on your behalf and process their payments accordingly. Please note, that depending on your insurance coverage you may be responsible for co-payments, co-insurance, or other deductible amounts.

If you have further questions, please contact our billing office or reach out to your insurance carrier directly.

We Accept TRICARE Insurance & More!

Payment Information

Payment is expected at time of service for all insurance plans. Patients have the ability to pay in full, pay by credit card (Visa, MasterCard or Discover) by the end of day, or set up a mutually agreed upon payment schedule with our billing department. Please note we do add a late fee for nonpayment of the insured’s portion of the charges on the date of service. Per insurance contracts, when the insured reaches their deductible amount, the insurance company will either pay for services in full or charge you a co-pay or co-insurance.
