A Patient-Centered Medical Home is a system of in which a team of health professionals work together with you, the patient, (and where appropriate, your family) to provide you with the best possible health outcomes. Your primary care physician leads the care team which may include nurses, receptionists, secretaries, specialists and other health care professionals. We will work together with you as the center of the care team.
We work as a team to make sure your child receives the best care possible
The patient-centered medical home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams.
Not every practice holds the title of a PCMH. Practices that earn the recognition, like our team has at Fairport Pediatrics, have made a commitment to continuous quality improvement and a patient-centered approach to care.
